How do I sign up?
Patients who wish to participate will be issued a MyChart activation code during their clinic visit. This code will enable you to log in and create your own username and password. If you were not issued an activation code, you use the “Request Access to My Account” button and fill out the request form on the MyChart home page. You will receive an activation code via email within 7 business days.

Your test results are released to your MyChart account after your physician has reviewed them. This is generally within 1-7 business days after your physician receives the final result.

Most test results such as labs, radiology and pathology will be released to MyChart within 7 business days of receipt by your provider as well as historical results since 2013. Due to the sensitive nature of some tests, those test results might not be viewable in MyChart. If you have questions related to result, please use the “Get Medical Advice” request to send a message to your provider and team.

Your MyChart information comes directly from your electronic medical record at your provider's office. Ask your provider to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

You will generally receive an answer within 2-3 business days from your provider or their clinical staff. Please note that MyChart should not be used for urgent situations, dial 911 if it is an emergency. Please contact your provider via phone if the situation requires immediate attention.

Yes, the date or type of records you are requesting may impact whether the records are available via MyChart. Most records after May 2012 will be available; however, records such as radiology images, photography, monitoring strips and records produced from other sources cannot be released via MyChart. Yes you can. This is called Proxy access with allows access to a child or adult UCM patient’s record. We have two methods to sign up for proxy access, if you are requesting access to a child’s chart you may fill out the proxy form using the “Request Proxy Access to My Child’s Account” form on our MyChart home page or inquire at your child’s next visit. To allow access to your account or request access to another adult’s MyChart account, please inquire about the paper form for Adult Proxy Access at your next visit. Adult to Adult MyChart access does require a signature from BOTH the patient and proxy.
Yes, you can. This is called proxy access and allows a parent (or guardian) to log in to their personal MyChart account and then connect to information regarding their family member. Complete a Proxy Consent Form and return it to one of our medical facilities to request access to this convenient service or select Share My Record from the Health menu. From there you can select "Friends and family access" and modify or add new access to your health record.

MyChart offers direct access to your personal health record and communicating about another individual's information would be placed in your health record. This information would not appear in the correct health record and could potentially jeopardize medical care.

No, due to the sensitive nature of medical information, each adult must establish their own MyChart account.

In accordance with UCMC policy, proxy access converts to a limited version on the child's 12th birthday and proxy access terminates on the child's 18th birthday
Birth to 12 Years of Age: Parents and legal guardians are provided with full access to their child's MyChart account. A week before your child's 12th birthday, you will receive an email alerting you that your access will be transitioned to a limited proxy access the day before your child's birthday.
12 to 17 Years of Age: Parents and legal guardians are provided with limited access to their child's MyChart account. A week before the child's 18th birthday, you will receive an email alerting you that access will be terminated the day before your child's birthday.
Children under the age of 18 are not permitted to have access to their own individual MyChart account.

You may click the "Forgot password" link on the sign-in page to reset your password online by answering security questions. If you are unable to change your password, you may contact the University of Chicago Medicine at 1-844-442-4278 to request a new, secure password.

Log into MyChart, go to the Preferences section and select the Demographics. Address changes will be verified and updated by UCM staff but mobile phone and email will be updated automatically in your record.

We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for too long, you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

You need access to a computer connected to the Internet and an up-to-date browser (such as Internet Explorer). Once you have created your username and password, you may go download the MyChart Mobile application for your iPhone or Android via the Apple store or Google Play store. Please note, you cannot create your account via the mobile application, it needs to be through a web browser session.

For your security, your activation code expires after 30 days and is no longer valid after the first time you use it. If you need a new activation code, you can call the University of Chicago Medicine at 1-844-442-4278.
